Proposed Change to Requisition Form

Dear Members

We have today received the attached draft Requisition form from the Chief Director, Ms. T Bezuidenhout.

Members are requested to peruse the proposed form and to provide us with your comments by close of business on 23 September 2020.

Ms. Bezuidenhout has indicated that the intended date of implementation is 28 September 2020.
